SOCIAL SECURITY
CARDS, for all of the adults
living in the household. |

BIRTH CERTIFICATES
or SOCIAL SECURITY CARDS, for all
minor children living in the household. |

MAILING AND
RESIDENCE ADDRESSES, acceptable
sources for address information are driver’s license, identification card,
PUD bill, or a printout from PUD. 911 addresses may be obtained by calling
the Sheriff’s office at 422-7200. |

RENT RECEIPT OR
LEASE AGREEMENT, if renting, or SUBSIDIZED HOUSING INFORMATION, if you live in subsidized housing or
are on section 8. |
|
PROPERTY TAX
STATEMENT, DEED, PAYMENT BOOK, OR TITLE,
if you own or are buying. |
|
FUEL BILL:
Your most recent bill or receipt for the most recent purchase of home
heating fuel. If the bill is not in your name bring a letter from the
person whose name is on the bill stating that you are responsible for all
heating costs. If you heat with propane or oil, bring a year’s history
of fuel purchases. |

ALL GROSS INCOME
VERIFICATION, received by ALL
persons living in the household who are 18 years or older for the 3 months
prior to application (12 month can also be used if you are over income using
the 3 month method.
Earned income
– All check stubs, payroll printout or employer’s statement
Self Employment
– Bookkeeping records and receipts for months requested.
Unemployment
– Checks stubs or a computer printout for 3 months.
TANF, GAU, PENSIONS, SSI, SSA, RETIREMENT,
CHILD SUPPORT OR ALIMONY – Award letters or documents that reflect income
amounts received during the months requested.
